SBR for metasticized colon cancer in lung

Anyone hae experience with this? Side effects? Any tips on how to handle it? Colon cancer had gone into the lungs and after 3 years has started to grow. The plan is to do SBR radiation and then chemo. Thanks.

There are a lot of people on this board who have had SBRT for pulmonary metastases, and quite a few threads can be found easily. However, each situation is unique, and there are several potential alternatives.

If you’re looking for general info, threads like those forwarded by Jacques may be very helpful. If you want the benefit of members’ nuanced reaction to your situation, it is important to provide information about your specific circumstances and questions, and optimal to provide a relatively detailed signature. You don't have a signature right now, however it appears from your previous posts that you are a Stage IIIA female colon cancer patient NED since completion of your Xeloda adjuvant therapy back in February 2006, but that you are now inquiring about SBRT for an 8mm lung met on behalf of a friend of yours who has metastatic cancer. Is that correct?

I had SABR recently for some liver mets, and I understand lung SABR is generally considered ‘easier’ than this as you have less GI upset to deal with (eg nausea vomiting). The hardest part is laying extremely still in a quite awkward position for a fairly long period (30+ mins) for each treatment, and doing the breathing exercises to keep the organs in the right place. Also the fatigue is REAL - I had never felt anything quite like that before (I would sweat from making myself a coffee). The good thing is you do recover pretty fast; within a couple of weeks a felt a LOT better, and within about 6 weeks I’d say I was basically over it. And the other good news is it’s usually very effective - everything they hit was dead on the next scan.

Rob: Just for the record, SBRT to the lungs is indeed quite a lot easier than what you describe for the liver. At least in my experience. While YMMV, there were virtually zero side effects. I believe there’s a short hit to WBCs (because they cycle through the beams), but then the immune system is often moderately boosted for at least six months (mine seems to have been). There was no fatigue, nausea, or pain. Perhaps there are issues if the targets are large, but others would have to chime in on that. I do agree about the breath holds being the most interesting bit. They let you pick how long to hold within a range, and I pushed it to get fewer holds, but you can find a sweet spot. No biggie once you get the hang of the machine. I will say that I initially misread your reference to “30+ minutes” as about the breath holds, and my first response was “Dang, they really are tougher in Australia.” :shock:

I had sabr to to 2 spots in the right lung. Did not notice any effect, the positioning and the staying still is the hardest. The radiation onc said there could be some rib pain because of the location of one of the spots but I felt nothing.
09/20:Diagnosed after colonoscopy for bleeding.
MRI and ct scan confirmed tumor at rectosigmoid junction aswell as lung nodules in RLL (1X1.2cm) and RUL (3X.5cm)
10/20: LAR tumour t3n2m1 4/24 lymph nodes.
11/20: pet scan confirmed Mets
11/20: folfoxiri - 10/6/21. 12 rounds in total
1/1/21 thoracotomy 1 met removed
2/2022 sabr to 2 lung nodule
09/22 PET scan progression of sabr’d nodules
10/22 phase 1 trial bispecific antibody
9/23 added pd1 blocker
12/23 bilateral VATS all visible (7) lesions removed
