mucinous adenocarcenoma

good afternoon, i'm from ukraine, i'm 29 years old, diagnosed on June 4, mucinous adenocarcenoma 3b, now completed the 7th course folfox / 12 tell me who faced this type of tumor? what am I missing in treatment? Maybe I need some other medications and DNA tests? sorry for bad english

Its a subtype of Colon Cancer, Many people with Lynch Syndrome present I think with this. Its treated about the same as other Colon cancers, but might be a candidate for Checkpoint inhibitors due to its MSI status.

1/16 Iri, Erbitux and likely Avastin (Trial) CEA going >.
1/17 CEA up again dropped from Trial, Mets growth 4-6 mm in abdomen
5/2/17 Failed second trial, Hospitalized 15 days 5/11. Home Hospice 5/26, at peace 6/4/2017

Even though you are MSS you may still have mutations that significantly affect your treatment, so you should still get it. And, not to confuse the matter but important, if your tumor mutation burden is 12 or so, you may still be eligible for immunotherapy. In addition, there are trials going on that combine other drugs with immunotherapy.

From what I've read, this could be particularly important to those with mucinous tumors.

In my opinion, a mutation test is not really needed unless you are Stage IV. If you ever become Stage IV due to a recurrence, then a KRAS/NRAS/BRAF mutation panel is needed in order to determine which therapy regimens will be available to you. But you might want to have the test done now anyway, just to see what your status would be. Also, you may want to do a more extensive mutation test that will provide a Tumor Mutation Burden (TMB) index, if these kinds of test panels are available in your area.

In the USA, the following therapy drugs are available for use by colorectal cancer patients. However, the ones in bold face type below will require the KRAS/NRAS/BRAF mutation tests to see if you qualify for them. You would qualify for all the rest except the ones for 'MSI-H only'. You don't qualify for the 'MSI-H' drugs since your tumor is not MSI-H.

2012 Zaltrap (Ziv-Aflibercept) - mCRC 2nd line,
2012 Stivarga (Regorafenib) - no restrictions
2014 Cyramza (Ramucirumab) - mCRC 2nd line
2015 Lonsurf (Trifluridine and Tipiracil Hydrochloride) - no restrictions
2017 Opdivo (Nivolumab) - mCRC 1st line+, MSI-H only
2018 Yervoy (Ipilimumab) - mCRC 1st line+, MSI-H only

Given that mucinous tumors don't typically respond well to conventional chemotherapy, genetic testing at an earlier stage seems wise to me. My oncologist has been having genetic testing done on all of his stage 3 patients for several years. It doesn't have to be Foundation One.
