Surgical opinion options?

Postby michellecairn » Tue Jan 26, 2016 12:02 am

At my doctor appointment 2 weeks ago I was questioning her about possible surgical options after I get my next CT scan. Originally she said she wanted to start with chemo to see if things shrink.

All we know for sure from my first scan in October and a diagnostic laparoscopy is there is thickening of the colon wall in the transverse colon, and that there are tumors on my perinotneum and on one ovary. So I asked about HIPEC, hysterectomy, and colon resection. She kinda looked at me crazy and asked me what I meant when I mentioned HIPEC. She actually told me she didn't know enough about it and that she'd research it.

So I go back and see her today and have my 6th chemo treatment. She said she looked into HIPEC and that depending on what the scans show it's definitely a possibility, but that she would want me to go someplace that has a lot of experience in doing the procedure. My scan is next Friday, February 5th, and I'll get the results on the 8th. I will have more chemo on the 8th, but I do know that I have to wait 6 weeks before any surgery because she's got me on Avastin right now. So I have some time for a surgical consult I guess.

My question is does anyone have any insights as to someplace in the Midwest - I'm in between Milwaukee and Chicago that would be a good choice for a surgical opinion? She said she doesn't want me at Froedert because the dr that specialized in HIPEC is leaving that hospital. I wasn't impressed with Froedert anyways because that's where I first had my initial onc appointment and the dr basically told me there would be no cure for me so I was not comfortable with him treating me at all.

Even if I had to travel further away I will do it - anything I can do to beat this I'm willing to try.

I realize these docs aren't near you, but -- after doing much digging on these forums and elsewhere -- these are the HIPEC docs we are consulting for my dad:

1. Dr. Sardi at Mercy in Baltimore, MD https://mdmercy.com/centers-of-excellen ... o-sardi-md

2. Dr. Levine at Wake Forest http://www.wakehealth.edu/Faculty/Levine-Edward-A.htm

3. Dr. Bartlett at U Pitt http://www.upmccancercenter.com/network ... m?id=21716

We also spoke with the office of Dr. Sugarbaker, who invented the surgery. He is in Washington, DC. His age cut off is 60 (or was it 65?) so my 70-year old dad was a no-go for him, but his clinical coordinator (who also happens to be his wife) was very helpful in helping me understand what I need to know and what my next steps should be as we consider HIPEC with other surgeons.

Our thought was if we are going to even think about considering such a radical surgery, we are going to talk with docs who have successfully performed said surgery many, many times. We have family near Washington, DC so that helped influence our Mid-Atlantic-ish line up.

I hope our HIPEC list will help your research and/or encourage others to ID docs in your area.

A family member of mine is fixing to go to Baltimore with a "difficult" surgery around appendix. I got that much out of her. Suppose to be 1 of 5 drs that can do it. I hope your #1 is hers. im sure its not HIPEC (unless she's not telling me) but I've seen a few surgeries around appendix on here lately. I think she's hoping for no surgery but I don't think they will leave even a non-cancerous polyp in there. I hope not anyways : ( good luck everyone.

I just want to thank everyone for all the links and information you have given me! I've been going through them all carefully.

I've just been frustrated because my cancer was found on a CT scan in the ER, and a confirmed with a diagnostic laparoscopy 2 days later as peritoneal carcinotomas which tested positive for signet ring cell in all the biopsies. They think it stemmed from the colon, and that's why they say my primary is colon cancer, yet no one has been able to say there's a tumor or mass in the colon. I think there may be since there is a spot where I have a twinge of pain if I press on my abdomen, but that may just be in my head ha ha.

My upper abdomen is distended, bloated, and swollen to the point I look 9 months pregnant. It shrinks a bit overnight, but depending on what I do/eat/drink I'm quite tight by the end of the day. It impacts my movement in bending and such. Not sure if it's coming from the spread on the perinotneum or what.

A lot of posts/stories I have read people are diagnosed and have surgery immediately and that was not offered to me. I was offered basically no hope by the first dr, and although I love the bedside manner of my current dr, I feel she may not have enough experience in dealing with colon cancer. My first scan after diagnosis is next week so I'm keeping my fingers crossed that I don't get really bad news.

I just want as much of the cancer out of my body if possible. I'm willing to travel to wherever I need to go, obviously financially it'll be a stretch, but hoping it'll be worth it in the long run. My dr will not authorize me to go back to work, I haven't worked since the end of September. I feel good enough to work on restricted duty and hours, but she's just saying I shouldn't work because of the germ factor.

Once again I appreciate all of the information and links. I hope everyone is doing well and keeping positive.
