Instead of fat-shaming, perhaps one might look into something like the percentage of people who have health insurance (and thus get colonoscopies). Or other factors.

Adding to Boxhill's comment, colorectal cancer rates within each state vary drastically from county to county. Economic disparities and access to health care are a big factor.

As we all know, obesity is just one risk factor among many. Highlighting obesity isn't any more informative than highlighting the amount of red meat consumed on average in each state.
DX: sigmoid colon cancer 5/2018. 48 F
laparoscopic sigmoid resection (24 cm removed); no stoma.
7.5cm adenocarcinoma -- mod. diff.
1 noncontiguous tumor deposit removed; 0/31 lymph nodes
T3 pN1c M0
5/18 before surgery, CEA 11.2
6/18 began FOLFOX
7/18: CEA 1.9; added neulasta post infusion
9/18: CEA 2.8

Not saying it’s not correct, but I think there’s a pretty strong correlation with median income in those states too. There would seem to be a correlation between income and colorectal cancer rates (fewer screenings, poorer insurance, lifestyle, diet, etc.), so there could be at least one other variable involved here.

correlation, with or without causation ?

I don’t see it as fat-shaming; I think Jacques has highlighted a valid and known risk factor for CRC. There are others - red meat consumption, alcohol, diabetes - I wouldn’t say we are ‘shaming’ those people either (I fall into at least 2 of these groups!).

I do think this is also way more complex that one factor of course - eg my comment earlier on income, also geographic access to care, and obviously genetics.

I agree completely buddy and I was certainly NOT making light of the situation.

Geographic exposure to any number of factors may well be a contributing factor to the
epidemiology of CRC incidence in this particular demographic.

I have actually seriously wondered if it actually is something in the water. I was living in a rural part of Minnesota when I was diagnosed, and had zero risk factors (thin, athletic, plant-based diet). And two of my neighbors were also diagnosed with CRC at a relatively youn age; both have since died of the disease. All of us drank well water.
Dx 12/2014 T3N2MX (distant LPLN) low rectal
12/2014-4/2015: FOLFOX (8 cycles)
4/2015-6/2015: 28 cycles of chemoradiation with xeloda, SBRT
8/2015: Robotic APR with iliac node dissection; path showed ypT0,ypN0 (complete pathological response).
11/2015 scans clear, CEA 2.1
11/2015 parastomal hernia repair
3/2016 CEA 1.7, scans stable...
6/2020 5 years of normal CEA and stable scans
Now dealing with pyoderma gangrenosum.
Totally disabled due to oxaliplatin induced neuropathy and dysautonomia

The same thing with the well water in one of our area neighborhood. We had a clothing factory that closed and was sited for dumping the chemicals into the ground. All the homes had well water....in the past 20 years, most of the residents developed cancers....from CRC, brain,lymphoma,bladder...So yes, it makes you wonder if it was the well water. I also lived in a rural area and use to run under the planes when they were dusting the crops. We had well water. My aunt lived next door and both of us developed CRC.

Actually my comment wasn’t even on your post! But anyway I think the watershed theory is fascinating. There does seem to be a clear divide, but I’m not in the US enough to know whether that divide shows up fairly commonly in other areas too (and hence may have another cause), or just in this case.

PS please continue to be a smart ass - I’m Australian and miss that here sometimes :-)

Hi ,
I have noticed people are being quickly closed down from discussing a healthy weight in the UK despite an upward trend . However our departing Chief Medical Officer has stated it’s a future issue that requires attention now to the point of proposing no eating on public transport !
The way I see it , it’s possibly only a factor but one that can at least you can do something about unlike your genetics so why not discuss it in a polite manner . If you at least know some of the contributing factors then you can address them or not as is your choice .
