Colon polyps

Postby MK1965 » Tue Sep 17, 2019 7:24 pm

This is my first post. I am 54 y/o white male, healthy, non smoker, non drinker, physically fit, 185 cm tall and around 215 lbs for past 15 years. I drink lots of espresso coffee. I live in Texas but notTexan and certainly loveBBQ and grilled meat.
Just recently, had my 2nd colonoscopy and multiple sessile polyps where found and biopsied.
Polyps were found in ascending and transverse colon in both flexures hepatic and splenic.
Got pathology report and 3 of 7 polyps are pre-cancerous classified as Sessile serrated adenoma 2 in hepatic and 1 in splenic flexure. Polyps were in size from 4 to 7 mm. Few of them were inflammatory polyps.
My 1st colonoscopy at age of 49 was clear, not found.
My GI doc wants to do colonoscopy in 3 years unless I develop symptoms.

My mother was diagnosed with rectal Ca in 2004 and passed away in 2007 from cancer related complications. My aunt ( moms sister) was diagnosed with colon Ca in 2013 and died in 2015.
I was diagnosed with prostate Ca at age of 51.
How worried should I be?

Hi,
There are few thing you can do.
1.) Lynch syndrome test, https://www.cancer.net/cancer-types/lynch-syndrome
2.) Change diet.
3.) Do a CEA test.

Nate provided some good suggestions! I would recommend a follow-up colonoscopy in 12-18 months if you are able to do so. With the potential genetic component plus the existing pre-cancerous polyps, I would not want to wait 3 years. My first colonoscopy in 2013 showed numerous polyps. My second colonoscopy in 2016 was 'clean' (which I thought was miraculous). I was told to wait five years. Exactly three years later I started having blood in my stool. Third colonoscopy found 4 polyps and 1 3cm adenocarcinoma. Most likely a polyp was missed in 2016. I don't say this to startle you... Just to encourage you to push for a scan more frequently. My niece just had a scope with precancerous polyps at age 30. Her doctor recommended one year follow-up colonoscopy.

Wishing you all of the best,
Lee

I think that I'd want some surveillance on shorter intervals.

You might consider the Cologuard test if you can only get a colonoscopy every three years.

NHMike,
Thanks for your response.
I do not have any problems with having colonoscopy every year if it is necessary.
My GI doc suggested next one in 3 years.
I am employed and have health insurance which covers colonoscopy once a year if deemed necessary with no out of pocket costs even if deductible was not met.
How it goes with Lynch testing? Should I request it or that comes as suggestion from my GI doc?

Good to hear. Lynch is a genetic condition which predisposes you to getting cancer. It also means that there are some relatively new drugs which can treat your cancer. I think that testing for it would be a good idea but a lot of people with Lynch get cancer a lot younger than you do which, I think, makes it less likely that you have it. But there is no harm in getting the test.

Aware, cautious, yes...worried, no. In other words, coming here was a good, proactive thing to do. Though number & type of polyps do matter in the risk classification, Bottom Line: How old was your mother @ diagnosis?

My first colonoscopy @ age 50 showed precancerous, inflamed polyp + others. I was symptomatic. 2nd colonoscopy 6 years later had polyps (was supposed to be 5 years). Third colonoscopy 3 years later was clear. Fourth & most recent colonoscopy on Sept 11 had two small polyps (6 years later, supposed to be 5 years once again). Instructions to come back in 5 years.

My understanding from my gastro doc in 2013 because I asked regarding my sons & stepsons: For a diagnosis of colon cancer in a parent age 50 or less, the general recommendation is minus 12 years from age of parent's diagnosis.

For me it is the age of my mother @ the time she was diagnosed w/colon cancer which was 86. So standard guidelines would apply to me, which would be age 50... & none too soon, obviously w/a precancerous polyp.
Because of ME w/precancerous polyp @ age 50, my sons should get colonoscopy @ age 38.

My husband clean colonoscopy @ age 40, St IV colon cancer age 47.
My stepsons should get scoped around 35. Unfortunately one of my stepsons was diagnosed & passed away @ age 35 of colon cancer.
Due to his father's dx under age 50, but mostly because of his brother's dx & passing, other stepson got scoped early, age 33, polyps, diagnosed with Lynch Syndrome. Another colonoscopy 6 months later, then annually thereafter.

However the standard has been changed to age 45 for screening, not necessarily a colonoscopy. If you have had radiation to the pelvic area due to prostate cancer, they recommend 5 years in the link below. But again, depends on an individuals total risk factors. It's a rather "loose" guideline in my opinion from American Cancer Society:

https://www.cancer.org/cancer/colon-rec ... tions.html

You are young for prostate cancer & it is considered one of many possible Lynch cancers (does NOT mean you have Lynch Syndrome). This is a good link on Lynch Syndrome (reader friendly):

https://my.clevelandclinic.org/health/d ... ome--hnpcc

Check with your insurance for genetic counselors & testing covered in your plan, whether you need a referral from family doctor or GI doc or your oncologist who is treating you for your prostate cancer. That's where I would start & go from there.
