Anyone pass out after abdominal cramping??? ER time

What a night!

My husband woke me up at 1:00 am this morning telling me that he was concerned about the severe abdominal cramping he was having. He said that it was in the area of his incision, mid belly-ish, and worse when lying down. We called the on-call doctor at MSK who reviewed his symptoms and said it may be prudent to go to the ER.

He is on day 7, now 8, of HAI FUDR. He describes the pain as being what he feels a few days after disconnect from Folfiri (last round of that was 4/29-5/1), which has happened after both of his two, so far, Folfiri treatments. Thus, we find it odd and worrisome that it was happening at this time in his cycles. I gave him some sodium bicarbonate in case he was having small intestinal gas before we left for the ER.

At the ER, he had bloodwork, adbominal xrays, and an EKG/ECG. Bloodwork shows low hemoglobin again, 2 points lower than last week, same with hematocrit, better MCV, elevated WBC and neutrophils, and low potassium. Cardiac and pancreatic markers were fine. EKG/ECG was normal. Xrays looked good so the doctor wasn't worried about obstruction. The nurse heard gas in his bowels moving around so I think the sodium bicarbonate loosened it up. The cramping went away while we were there.

Here's the distressing part: Before the nurse drew his blood, his BP was 129/81. Awesome for him! Then, she drew five vials and his eyes widened..."I hate needles" etc... He started to feel nauseated, retched, and his BP bottomed out. He also went unconscious for a moment. BP was 87/51. They called "vagaling" and he had "vagaled." Within a half hour to 45 minutes after the blood draw and vasovagal syncope, it went to 112/63, low for him. Since everything was looking okay, he was discharged.

Then we get home. He walked slowly up the stairs, walked to the bathroom, and then fell unconscious again. He came to, after a few seconds, but I was a bit hysterical and called 911. My husband said he was fine, it was just from walking up the stairs.

He drank a sugar-free gatorade and I gave him a potassium tablet.

He went to bed in the bedroom and he is sleeping. I am so worried. I don't think it was normal for him to pass out again. I am calling Dr. Kemeny's office in a couple of hours.

CEA 5.8 idk wtf is up with that, but everything else is clear!
CEA 3.7 make up your damn mind...
CEA 1.5 that's a new low!

He had a little sleep. He still is getting lightheaded whenever he stands up from a lying position. Seems odd that this is still happening. No answers from Dr. Kemeny's team. "Stay hydrated and take it easy" is all they said.

How is his blood pressure, is it still low?

Dear S, what a scary experience! So you did not get any answers from Dr K as to why this could happen? Is it maybe a side effect to the chemo or? Sending hope that your DH will feel better soon! and for you to not have to worry about this happening again.

No answers. I had to call back this afternoon because he was still symptomatic with lightheadedness and was unsteady on his feet. I wanted to let them know about this even though I had no clue how important it was or not. Dr K did not like how it sounded at all and insisted that we get back to a hospital for IV fluids. We returned to our local hospital where I dropped my husband off at the ER. I had to go home to tend to our dog and a few other things and when I returned to the ER I saw my husband violently vomiting. He essentially threw up all the fluids he had ingested. Very concerned, he stated very firmly that he intended to have them admit him as he was not interested in being in and out of the ER multiple times.

His red blood cells keep dropping and now his creatinine is below normal. The last I heard, they are waiting on the results of a CT scan. Fortunately, we have a good friend coming up from New York City who should be arriving any minute. He happens to be an EMT. I have been trying to sleep for the last hour but I am worried sick.

EMT friend, Who is with him right now, says that our local hospital is transferring him down to New York City (MSK). I don’t know why but I assume it has to do with the results of the CT scan.

I know it’s very difficult to do but please try your best stay calm and strong. Sending you and your husband positive thoughts. Hope he feels better very soon.

At MSK now finally. They brought him into interventional radiology to do possible embolization depending on what they find. They will also maybe put a stent around the catheter because it looks like the catheter is leaking. I honestly don't know if I'm separating things or mixing things up right now. They will keep me posted and consult with me once they go in and get a good picture of what's going on via angiogram.

I have never seen him in so much pain in my entire life. Essentially what’s going on is that chemo is being leaked into his abdomen.

holy crap, that sucks!!! I feel for you both. I hope they get things straightened out ASAP. Seeing someone you love in that kind of pain is a hell of its own.

My husband has vasovagal syncope. There's a very straightforward test called a tilt table test that will diagnose it, if your husband may have it. It sounds like that's probably not the main problem in his case, though, and a bunch of other things are going on. You are both in my thoughts.
DX: sigmoid colon cancer 5/2018. 48 F
laparoscopic sigmoid resection (24 cm removed); no stoma.
7.5cm adenocarcinoma -- mod. diff.
1 noncontiguous tumor deposit removed; 0/31 lymph nodes
T3 pN1c M0
