Need suggestion on stage 3c to metastatic to lung

My husband was diagnosed as colon cancer on Oct 4, 2018. Did a surgery for colon and removed 30cm colon and 6 lymph nodes, where 4 out of 6 were cancer cells and said its stage is 3c. His CEA reduced to 2.2 from 19.5. We did chemo with 8 round of cycle and the last one was on May 10, 2019.

After three month of rest period, on Aug 10, 2019, on that CTS showed some nodules at lung few at right and one at left lung. Doctor referred to lung surgeon, thought it might be metastatic colon cancer. Lung surgeon decided to do surgery on right lung to get small two aggressive nodule and did on Sep 6, 2019. Now got a biopsy confirmed that it is metastatic colon caner, though his CEA level is decreasing to 1.5. When we was diagnosed for first time it was 19.5. Now doctor decided to give again next round of chemo with same dosages as he used to take, Oxaliplatin once three weeks and Xeloda two weeks and one week off for again 6 months of chemo. Then again left lung surgery.

Now I am worried that looks like same dosage doesn't work that why cancer cell is increasing and if he takes more dose he got side effects like neuropathy and skin peeling. Also is it good to do left lung surgery and then chemo or do chemo for 6 mths and surgery. Did any one face this issue. What are your suggest in this condition?

09 thoracotomy at right lung took two nodules, still left at left lung.metastatic to lung
10-Started CAPOX again
10/31/19-2nd cycle Allergic to OX taking CAP oral chemo alone for 1 cycle, CT scan showed left nodule inc. from 5to6mm
12/19 ct scan ndoc visit 7mm. Doc want to do 3 cycles of CAP
2/18 pet n ultrasound endo confirmed metastatic to perirectal 4mm
3/16 CT new nods at r/l lungs, inc pelvis

Hi Suniva,
My husband had multiple small lung lesions on both sides, too many for surgery or radiation to make sense. He did 12 cycles of Folfiri + Avastin and they're not visible anymore, so that could be an option for your husband if Oxaliplatin stops being effective (it didn't work as adjuvant chemo for my husband), and/or if neuropathy becomes an issue (which it was - and still is - for my husband). Suniva - My husband is in a similar position and has what the doctors think are mets to both lungs. The lung surgeon said it can be harder to find the met after chemo if it shrinks, but it seems like after a discussion with my husband's primary colorectal surgeon and the oncologist, they are still leaning towards doing chemo first. We have scans next Monday after a break from chemo to see how much / if they grow. His CEA has actually decreased since he's been off of chemo, so not sure what the scans will show.

Claudine - Since Folfiri / Avastin worked so well for your husband, are the doctors planning to keep him on that combo or are they going to give him a chemo break or put him on maintenance chemo?

He's going to do another 6 cycles (every 2 weeks, so another 3 months). He still has a met in his adrenal gland that's slowly shrinking, plus his onc wants to really go after possible random floating cancer cells that can't be seen through imaging (including possible remnants in his lungs). He's tolerating the regimen quite well. Hopefully once that's done the adrenal met will the only one left and they can go after it with radiation or surgery!

That's great Claudine! How many lug nodules did your husband have? My husband has ~8 <1cm nodules between both lungs. It's great to hear that chemo worked so well on your husband's - hoping we get similar results if that's the path we end up taking.

We never got a number - just "multiple" on both sides. The largest one in March was 5mm and there were a lot that were really small - 1mm. Too many to do anything but a systemic treatment... I hope your husband responds well to chemo!

Thanks for sharing Claudine, it was a relief that chemo works for small nodules at lungs. It's great that chemo worked so well on your husbands and hoping similar results for my husband.
