Polyps and Procedures

Hi All,

I am new to these forums....

I am from Scotland/UK , the medical terms and ways are slightly different but I have seen a few of the feeds and some amazing people with amazing advice and strength....

My background is;

2010 = Age 37, I was diagnosed with Stage 3 aggressive Breast Cancer called Her2+ and had a mastectomy, 18months of Intravenous treatment with 5years of tablet form after.

Fast Forward to (May 2018) = (age 45) had a lot of bleeding, went to GP and told that the UK testing age was 50 for colonoscopy. ... I remained persistent, with numerous appointments, eventually I seen the main DR (November 2018) and straight away he gave me a QFIT test to use on my stool. These tests are accurate in ruling out unnecessary colonoscopy for piles etc and it locates occult blood (hidden)

My results came back positive, so I was then referred for colonoscopy.

2019 (age 46) = colonoscopy was 2weeks ago (Feb 2019) it was extremely painful, so much so I screamed out, I also have a retroverted womb (sits on bowel causing pressure) along with endometriosis..... not sure if it was scar tissue causing the pain, just felt burning and sharp pain going from ovary to ovary.

Once he was past the initial insertion, he located ;

* Distal Sigmoid Area = 1 x Pedunculated Polyp (1p) 10mm and removed this for pathology

AND

* Rectum Area = 1 x Sessile (Flat) polyp (IIc) 12mm - was unable to remove via colonoscopy, took two biopsies, one at base and one higher up. Requested I am back in within 3weeks to have this removed via Sigmoidoscopy - date set for 15th of March.

I had klean prep prior to colonoscopy and to use again next week, I was sick the last time, hopefully ok next week (fingers crossed)

I have made an appointment with my GP for Friday this week as I was unsure how long the pathology results take or if I'll be told before my sigmoidoscopy or after ?

Has anyone had a pedunculated polyp and /Or sessile at the above size or there about and had pain like I did at your colonoscopy?

(I was prepared for being 'uncomfortable' due to the air but not actual physical pain) which now has me a little anxious about the next part with sigmoidoscopy etc, and I've had numerous procedures and major operations over the years for other things but this is the first i've had extreme pain during a procedure.

I was also given the sedation, but it did not work and I was able to watch entire check of colon and the biopsies and removal (while gripping the bedside bars)

When I get further results I'll keep you updated.

Depending on sedation, sometimes I am able to enjoy watching colonoscopy on a screen and listening to discussion on a progress and challenges of a procedure, but sometimes I am completely knocked out.
Next time just tell about your previous experience and ask about higher dose of sedation.

Pedunculated is good.
10mm is good.
Just wait for a pathology report.

Sessile is a 2nd best option.
12 mm is good.
Removal via colonoscopy/sigmoidoscopy is good (much better than laparoscopic or open surgery).
Just wait for a pathology report after this polyp is removed.

I see only pros and do not see any real reason to be concerned in your post.

It seems that everything was caught on time.

Bless, thank you so much for your positivity, I had a chat today with my local GP, they said no results as yet and the specialist will advise further when I see them next week.

GP agreed most likely my endometriosis was attached to bowel hence the intense pain when initially started but feeling better now about (part 2) my procedure next week for the sessile polyp and shall definitely take your advice and ask for higher dose if possible .

Everything else aside, I actually found it interesting to see (I'm bizarrely ok with things like that )

Thank You again for the information about the different polyps located, very helpful to know and shall add an update once I have all the results and chat with the specialist.

Hi,

**Update from my last post**

Had Sigmoidoscopy today.

The 10mm pedunculated polyp that was removed at colonoscopy was benign.

The 12mm Sessile (Flat) polyp removed today has been sent to pathology, once they removed it, A small further residual polyp was located and destroyed with a  'cautious hot biopsy'

The previous biopsy taken for (sessile) confirmed the following;

'TVA with low grade dysplasia' and also today confirmed as (Is, pit type IIIL 12mm)

I have no idea what most of that means, Although I do know the following from specialist .

1st polyp removed is benign Yay

2nd one still needs the further pathology results to 100% confirm,   they need to test and see how far down it was rooted and overall conclusion etc

In the meantime, I need to go back in 6 months for sigmoidoscopy for further biopsies at that same site.

Will only be back into hospital sooner if pathology results state otherwise

Pathology results are around 6 weeks this time.

All in All, so far so good

FYI - Information on pit patterns:

Kudo et al first highlighted the feasibility of applying the “pit patterns” to distinguish neoplastic and non-neoplastic polyps via magnifying endoscopy. Kudo et al classified colorectal polyps according to their appearance, structure and staining patterns.

• Type I pits appear as roundish pits;
• Type II pits appear as stellar or papillary pits;
• Type III-S pits are small roundish, tubular pits (smaller than Type I) and
• Type III-L are roundish and tubular pits (larger than Type I);
• Type IV pits appear as branch-like or gyrus-like pits and
• Type V pits appear as non-structured pits.

Type I and II are considered benign changes (e.g., normal, hyperplastic, inflammatory polyps), whereas pit pattern classes III-V are considered to show neoplastic and malignant changes.
