New plan for para aortic lymph nodes. No surgery, radiation instead

I would like to start this post by commenting that I've recently seen many new people posting with lymph node metastasis. I hope that this post helps those people. Yesterday my husband met for the second time with Dr Bartlett at the Hillman Center. We were told that a multidisciplinary team of some of the world's best oncologists met and decided radiation would be the best course of action to treat my husband's para aortic lymph node metastasis. This was decided because there were three lymph nodes along the chain and not just one. They feel that with radiation a larger area could be targeted for treatment, assuming micrometastassis exist in other nodes. He will continue some sort of chemotherapy we are not sure yet what that will be until we meet with our local radiation oncologist later this month. My husband is very happy with this plan because it means he doesn't have to have surgery, but can still expect a positive outcome. The doctors are very excited for my husband and really feel like this will give him at the least another long-term period Of remission. The radiation oncologist mentioned that the type of radiation used would be sbrt therapy. If I have this all straight this is really new territory for colon cancer. He told us that the treatment was developed at UPMC magee-womens hospital for initial use in gynecologic cancers and treating it for colon cancer is new territory. For that reason I will share the doctor's name with you in case anyone is interested in contacting this doctor for this sort of treatment. Wife to DH/ Father of 6 (age 42 at dx) diagnosed Jan 2015 stage IIIC
Tumor deposit in mesentery 13/24 lymph nodes +
CEA at dx: 5
MSS
Low Grade/Mod. Diff.
FOLFOX 6 months
N.E.D until June 2018
PET Scan 6/18
Biopsy confirms cancer in 3 Paraaortic lymph nodes
Folfiri + Avastin (6tx) and Xeloda during radiation
Cancer all over both lungs dx Jan 2019
FOLFIRI +AVASTIN presently

6/28/19 Reversal and port out
CEA 2.1; 1.9; 2.6; 2.8; 2.3; 2.4; 3.0; 3.4; 3.1; 3.4; 3.0; 3.1; 2.6
Latest update: 8/21/23 Clear CT with CEA 2.6!

Me: 34, first colonoscopy 11/16/18—normal! Come back in 5 years.

Thanks so much for the update and the helpful information. This sounds like it has a good chance to provide long term remission.

It also seems through anecdotes on this board that chemo can wipe or suppress those micro mets if they are present in other nodes.

If you want a great story about radiation and lymph node mets - search for the user “ TYtiff (I think). They don’t post anymore after being clear for 5 years.

Pathology T3N2bM1 19 of 24 Nodes Positive + tumour deposits
PET scan - Para-Aortic and Iliac Lymph node spread. Stage VI.
Moderately differentiated. MSS. KRAS/BRAF Wild.
Mutations: TP53, ERBB4, MLL3, PDCD1LG2, PRKDC, SMAD3
FOLFOX + Bevacizumab Commenced Jan 9/2017 PET Scan July 2017 - on maintenance 5FU/Bev every 2 weeks.
Progression after Covid19 induced break June 2020. Resume Maintenance chemo of Capecitabine and Bev

Thanks for the update. My husband has same nodes and had SBRT as well. Waiting on follow up scan. Best of luck and positive thoughts!

Stage IIIC Rectal Cancer with regional lymph nodes
FOLFOX 6 months
Chemo Radiation Therapy
Complete Response to treatment - Watch and Wait
NED March 2016 - Feb 2018.
PET Scan March 2018 - single 1cm Para aortic lymph node
Xeloda/Avastin- 2 months, Fail.
2nd opinion at MSKCC suggested SBRT
SBRT July 2018
August CT showed original lymph stable with two additional Para aortic lymph nodes 7mm each
CT scheduled for Oct 2018
