TATME

Anybody have this surgery done recently anxious to hear form anyone that has!!!!
I must meet my surgon on the 20th to discuss this proceedure which he wants, to procced with on the 3rd may via a new surgon
Im extremely anxious as to the proceedure and longterm outcome

CEA 4.4
DX T3 L3C M0 2.5/3 cm above AV.
JAN 3RD started 1650mg Zelda 2xday, with 28 radiation
Did tagamet 800mg daily and 75mg IV VIT C WEEKLY UNTIL SURGERY and
Tumor reduce by 80% 1 LN still remaining
TATME May10th, temp illeostomy
10/07/2018 CEA 3
MMR INTACT
Began FOLFOX July 10th
24/08/2018 Allergic reaction so next infusion lucovorin and 5fu
CEA 4
Second attempt with oxi aug 12th

No help here : I had an open Total Mesorectal Excision via low anterior resection.

taTME ( transanal TME ) has been studied as an alternative / comparative to the more traditional
laTME ( laparoscopic TME ) and is a more technically demanding combination procedure ... AFAIK.

I believe like ANY procedure :
the experience, expertise and success rate of the individual surgeon/team is more important than the specific technique

JMO ... BUTT a few starter questions might be :
how many of these have you done ?
what is your success rate ?
what is the complication rate ?
what is the long term prognosis ?
will you be doing this yourself ?
why is this technique preferable for my specific situation ?

Ive googled the procedure so I kbow what lies,ahead, apparently it will b 2 teams, one top one bottom lol
But it's an old proceedure, but now seemingly being resurrected as a proceedure but cant find much on outcome as no actuall long term studies were done???
As far as im aware, full TME has better longterm oncological outcome, but im nust anxious as no one seems to has. had it done, im also very determand to preserve sphincter mussle for reconnecting after...

My CRS ia a professor, and the guy doing the TATME was,his understudy so well known to one another

Surprised ireland is forthocomming in new innovative procedures :shock:

Im confused as to why u had a TME, if u had CPR, to neoadjuvent treatment????
I though u skipped surgery and went straight to mopup chemo???
Ive obviously missed somthing in ur journey

I had pCR confirmed by pathology on the resected tissue after surgery, NOT cCR based on clinical assessment or imaging before surgery.

Standard of care for rectal / low rectosigmoid (2007) involved neoadjuvant chemoradiation, resection, adjuvant chemo ... which is what I had.

Makes u wonder about the need for radiation if they are going to prob remove the complete rectum anyway during the SURGERY??? And then predisposing you to an ostomy ALSO!!!!!!
A lot of peoples problems long term seem to stem from radiation,
As majority will always need surgery afterwards anyway
Im happy that they are doing a TME, as I feel ill b happier about less worry if reassurances, unless theres a downside that im unaware of????

Radiation is the gift that keeps on giving !
BUTT ...... just fyi !
it allows lower doses of chemo to be used concurrently
it can be used to downsize tumours pre-surgery
it "sterilizes" the surgical field wrt cellular infiltrates, and
currently ... it can produce a cCR result which for some patients, in some situations
allows for an aggressive watch and wait follow up without full resection.

I suppose ur right but for many including yourself its, once its,removed u realise the responce.....

Im completely taken a back that no one has had this taTME. Ive looked at other forums too and nothing!!!!
I may start a thread when its,all over next month so anyone considering it can make an informed decission and a human to relate to and not just paper studies
Ill know friday exactly what the peoceedure will b,
Its ultimately my decision, but I think ill run with their option,,,,
Ill have a SERIOUSLY sore ass tho...... lol from every angle :lol:
