ADAPT and Celebrex - What happened?

Several years ago there was a lot of excitement about the ADAPT protocol and using Celebrex to target stem cells. As far as I can tell the ADAPT study is now over, and no results have yet been published, and there's hasn't been much news about using this cocktail in a while.

Does anybody know what happened? Did the study fail? Were the other issues? Is anybody currently using Celebrex+Xeloda?

Really good question. I don't have an answer to the piece about the study but I do know there were a number of people here doing the protocol. Pretty sure several of them are still on it. Check out the profiles for Sophy and DBF, both of whom are Stage IV and longer-term NED. I think Betsydoglover did it as well (but has now finished?). I've been taking the Celebrex part but not the Xeloda (any more).

Bottom line--I've been on this board several years and from what I've seen, a significant percentage of patients do very, very well on this protocol. I wouldn't let the lack of recent information deter you from pursuing it as an option. Hopefully someone else will chime in with more info. You might try PM'ing Maia. If anyone here knows, she probably would.

I emailed Dr. Lin directly to ask for his advice and guidance on taking the Xeloda/Celebrex combo. He did respond that he is traveling and will answer me when he has a chance. Will report back what he says. I just found it curious that the study seemed to have abruptly been suspended.

I'm interested as well. If I stay stable I'd like to consider ADAPT going forward. Please keep us in the loop when Dr. Lin responds.

I have been on ADAPT protocol of 1000mg capecitabine and 200mg of celecoxib on a three week cycle since August 2014.

I started when I got home to New Zealand three weeks after my third lung laser thoracotomy surgery in Europe which made me to NED.

When I started this regime my oncologist in New Zealand contacted Dr Lin to ask him the dosage and duration. He replied that I should initially take for three years the dosage I am on.

All my scans have been clear since I started ADAPT.

I contacted Dr Lin by email a couple of months ago to ask him how the trial is going and that I am doing well on it and how long does he think I should keep taking the medications

He replied that he is very pleased to hear from me and glad I am doing so well but that is all he said and he hasn't replied to any more emails.

So my oncologist and the head oncologist and I all think that it will be safest for me to continue Adapt indefinitely until it stops working or something better comes along. I have already exceeded my oncologist's estimate of my life expectancy but she seems to be happy to be proved wrong.

Dr Lin called me in response to my email. He didn't get into why the ADAPT study had been closed, but mentioned he is planning a new study. He still believes very much in the benefit of this cocktail.

What is your exact protocol? My local oncologist is considering starting me on this program and we are collecting the different dosages that we have found people to be taking. You are on 1,000 mg of Xeloda once? a day and 200mg of celecoxib once? a day for a week and then off for 2 weeks. Thanks for sharing!

Is it common for people who are NED to stay on chemo indefinitely? I've never heard of this before.

Cynthia - so nice to see you are doing OK. I take 1000mg of xeloda morning and evening for two weeks then one week off then start again. I take 200mg celebrex morning and evening every day, no break. I also take 400mg cimetedine twice a day although this is not part of ADAPT. The xeloda dose is low compared to usual dosing but in order to tolerate it long term (50 rounds so far) the dose needs to be low. Hope this helps.

Elan - my oncologists think that it is likely I still have cancer cells but that they are being prevented from growing by the ADAPT. As my quality of life is good it seems unwise to take a break from chemo if I don't feel the need for it as then the mets would grow which would be very difficult/impossible to eradicate.

The usual reasons for a chemo break seem to be either the chemo is no longer working or the patient's health is suffering too much from side effects - and I am not in that situation at the moment.

Also, CEA and ca19-9 are not markers for me so it would only be when a met was big enough to see on a scan that it would be known about. My treatment is mostly paid for by the New Zealand public health system so I do not need an insurance company to approve it.

I am pleased you heard from Dr Lin, I too have been looking for updates on the ADAPT trial but not found anything recent. Would appreciate your posting if you get any information.

Bumping this up. Wondering how those on Adapt are doing? How do you convince your oncologist to give this a try? I'm fearing that just xeloda is not enough to keep me long term NED. Thanks for chiming in!

I have had one new lung nodule pop up since starting ADAPT almost 2 years ago. I had a cryoablation last month for that 6mm nodule and will continue on ADAPT indefinitely for now. Doing well and living an (almost) normal life. NED again!
