Any one have guidance for stage 4 colon mets to PC? Scared .

Hello every one
My husband is 36 and has been diagnosed with Stage 4 colon MSI Stable, Signet Ring Adenocarcinoma.
He has only completed 6 rounds fulfox at which time they did a laparoscopy to determine candidacy for HIPEC.

They deemed his disease too advanced.
I really would love any guidance, stories, or thoughts.

Hi. Sorry to hear about your husband. Nobody likes it when someone is newly diagnosed. I would push through with HIPEC if that is a viable option, followed by chemo. You may also want to look into immunotherapy trials for MSS colorectal cancer. You will find there are a number of individuals who were diagnosed with peritoneal mets who are surviving. Best of luck.

12/2014 - 6/2015 FOLFOX + Cetuximab
10/2015 - Recurrence liver
Liver resection 10/2015
FOLFIRI 11/2015 - 5/2016
Recurrence liver, nodes 11/2016
Pembrolizumab started 12/2016 -> pseudoprogression(?) -> biliary obstruction -> biliary stenting
Chemo 4x: most mets inactive and smaller on PET-CT
March 2017 - Back on Pembrolizumab again
Sept 2017 - SIRT - > NED
2019 NED

2 Liver ablations 1/2016, peri mets
Y-90 3/2016 & 4/2016
Failed HIPEC 6/2016, metastasies to spine, additional liver mets, peri mets increased significantly
Folfiri 8/2016
Scans 10/2016 revealed shrinkage, jaundice, chemo reduced
Scans 12/2016 revealed growth, hospitalization
Biliary drain & stents placed 1/2017 but jaundice persists

I am sorry to hear of what you are going through. Make sure you use the search tool on the top left corner under the "quick links" tab. This will allow you to read all conversations related to your circumstances. Also, many on this forum, and I agree, say to get a second opinion from a major cancer center depending on your geographic location. ie: Sloan Kettering in NYC, Dana Farber in Boston, or Anderson in Texas.

I have heard, and have witnessed, that the second look from these centers allows for a patient's peace of mind that your local oncology plan is the same as the big city plans, and if not, you can decide what is the best plan to follow with the most recent advances in fighting this cancer.

D/H 47 years old, 10/2014, Stage IV M/CRC, nodes 12/15, para-aortic, 5 cm sigmoid resection, positive Virchow. KRAS mut, MSS, Highly Differentiated, Lynch Neg, 5FU/LV and Avastin 1 YR (Oxi for 5 months), Zeloda/Bev since 01/2016. 02/2019 recurrence para-nodes, back to 5FU/LV Oxy/Bev. It is working again. Are you saying that it has been determined he is not a HIPEC candidate? And the primary in colon hasn't been removed?
The microsatellite status (MS) is MSS, then. Any other mutations?

I really hope you get a second opinion. My boyfriend was in a similar situation. He only had peritoneal mets, he did FOLFOX for 12 Rounds and then the first surgeon said no to hipec after doing a laparoscope. Same reason. Too much disease. We were heart broken but wanted another opinion to atleast get more questions answered and some closure. Another surgeon was confident she could do the surgery and did. The surgery went well and she was able to get it all.
DH dx. Stage 4 Colon cancer with Peri mets Dec '15 @ age 29
12 Rounds FOLFOX & then successful HIPEC in 2016.
Diagnosis changed to appendix cancer.
One small recurrence to pelvis 9 months later.
Resumed chemo and staying stable.
