Ablation of lung mets

Postby AngelaN » Sat Aug 15, 2015 4:43 pm

Hi,
My DH has 5 lung mets. One is close to the mediastinum, and has just grown since his last scan in May to 1.2 cm and is going to get SBRT. He has 4 more all about 2-4mm in the periphery of his lungs - 2 on each side. What is the smallest size met they can ablate? Would be nice to get the rest of them treated.
DH diagnosed with Stage 4 CRC Nov 2013
11 tx Folfox/Avastin; LAR/liver resection/introp RF of liver - May 2014
3 treatments FOLFIRI
Lung resection - Sept 2014
FOLFIRI + Avastin x 10 treatments
Avastin+5FU q3 weeks x 4
SBRT x 1 lung met
RFA for remaining 3 in Oct and Nov
SBRT for liver met Jan 16
lung mets growing
Enrolled in NIH TIL trial Oct 16
XRT for lung met 12/16
Cells didnt grow for TIL trial Feb 17
Waiting for NCT03085914 at Penn

I just had RFA for lung mets number two, I'm not positive but I believe they like 1.5 cm so that have something to target also less than 3cm seems to be preferable. If it were me I'd skip the sbrt and go for ablation the side effects are anything but benign.

Its in a spot that no one can get to easily. The IR guy said for sure he would get a pneumothorax and everyone agreed for this lesion SBRT is the right option - even the surgeon who was concerned he would have to take the whole lobe (which would be a bad thing since he had his other upper lobe removed last summer. I just want these other 4 gone. And unfortunately since they seem to be growing on maintenance chemo we may have an option sooner rather than later to get them fried.

So there you go, seems to be the best option. I believe a single nodule being radiated is better than doing a small group. Good luck to you and your husband.

Angela, you may find this interesting.

No sweat, I'll bet you've read all of them already. It's the results of me fighting with the insurance company. When do you anticipate #2 ablation?
