When Does the Survival Clock Start?

I just scheduled my first scan after completing treatment so I am pretty anxious and can't stop thinking about things. One actual question came to my mind:

Given that survival statistics are time conditional, when is the starting point for calculating survival time? I searched and read 2 different things on the internet:

1 - At the date of surgery.
2 - At the end of chemo.

Any thoughts or opinion?
35 yo Male
Rectal Cancer IIIb
28 rounds Radiation + Xeloda
TME LAR & temp ileostomy 11/19/2014
yp stage IIIa - 0/19 LN's T3N0M0
FOLFOX - 8 rounds, completed 4/17/2015
Reversal - 7/15/15
CEA: 5.0 pre-treatment; 1.2, 1.3, 1.3, 1.0, 0.9, 1.1, 1.5, 1.3, 1.6 (Change to Roche vs. Bayer)
Scan:10/13/15 - Lung "spot" indeterminable watch and wait (later determined to be a cyst)
Scan: 4/18/16 - Clear
Scan: 4/12/17 - Clear
Scan: 2/26/18 - Clear

I was told it starts at the time the main colon tumor was removed....but then others are told differently.

I'm pretty sure it starts counting at time of diagnosis . . .
Dec 2012 - CC 2 unresect liver mets, CEA 41.8 (MM 65yrs)
Jan 2013 - colectomy @ spleen 2/26 nodes IVa T3N1bM1a
Feb-Jul - 1x Xelox-7x Xelox/Avastin, shrinkage from #3
Aug - 2x PV embolization (both failed)
Sep 2013 - R liver resect, 25d hosp (liver failure/delirium, lung emboli, encephalopathy), no living cancer (pCR)
2014/15 - recovery, scopy: 2 polyps
2016 - new town/life
2018, scopy: 2 polyps
2018/20 low (1.0-1.4) CEAs/clean CTs: 4x2014, 6x2015-17, 3x2018-20
next June 2021!

Time is always counted from the time of dx - onset of symptoms is also important. Keep in mind, that after you have metastases, your clock start again - now from the time of the dx of mets. So, if you see stats about mCRC they will be at 5years survival from the time of mets dx.... Do not look at stats, it is personal, not business :P

Survival time starts when you start living as though it doesn't matter.
7/11 diagnosed Stage 2 colon and rectal cancer
chemo/rad
lar/temp ilio
Reversal & port removal
21 round of chemo Folfox 9tx, 5fu 12 tx
Last treatment July 2012

According to the ACS, the 'clock' starts at the time of diagnosis.
According to many oncs....it starts at the time of the last treatment (whatever that treatment may be)

In the nine years since my diagnosis, those are the two most common times that I have heard of.
Dx 6/22/2006 IIA rectal cancer
6 wks rad/Xeloda -finished 9/06
1st attempt transanal excision 11/06
11/17/06 XELOX 1 cycle
5 months Xeloda only Dec '06 - April '07
10+ blood clots, 1 DVT 1/07
transanal excision 4/20/07 path-NO CANCER CELLS!

Inherently the survival clock starts from when you are diagnosed.
However if you are looking at the idea of "xxx years to cure" then it is measured from the last medical intervention, be that surgery or some sort of radiation or laser surgery.

At MSK it's from your first clear scan. Usually after surgery to remove tumor.
DX Jan 2010, at age 47
Feb - colon resection - 2/17 nodes positive
April - liver mets - Stage 4
3 months Folfox chemotherapy
August '10 liver resection and HAI pump
7 months chemo FUDR HAI and Folfiri systemic
NED since August 2010
Last treatment April 2011
HAI Pump removed Dec 2015

My onc told me it is my surgery date. When tumor was removed.
Dx@45 stage t3 n1b m unknown IIIB
Resected in 8/2010 rectosigmoid
5.5 tumor with 3 /26 lymph nodes
Lymph vascular invasion
Folfox sept 2010 to feb 2011
10 tx only stopped due to low wbc

I asked my Onc. He said first scan after chemo. Butt! Since I had a PET scan a month after surgery and was NED I'm going to use surgery date. I don't care what he says. :)

Very cute answers some of you, but the distinction is actually an important one as far as I'm concerned (although I hate the term "survival"... It should be "non-recurrence" to distinguish from overall survival). My diagnosis date is about a year prior to my last treatment date. Since the largest percentage of recurrences happen the first two years, are my two years up or do I have another year to go before I can breathe a slight sigh of relief? The distinction does make a difference because of how the stats are tracked with patients who have gone before us.

My port surgeon wanted to wait until 6 months after the end of treatment to remove my port. I asked my onc what the big deal about 6 months was and he told me that 30% of recurrences are discovered at the sixth month point. It seemed to be understood by both my onc and my port surgeon that this was 6 months after the end of treatment. I think I have another year to go before I am past the two year mark.

Patients who do not have chemo would have their nonrecurrence tracked from the date of tumor removal because that would be the end of treatment for them, so the tumor removal point is true as well for these patients if stats are based on the end of treatment.
