Today CT scan showed progression: slight increase in dimension and number of pulmunar mets, both lungs; pleural involvement and what's worst RIB involvement (directly by the pleura touching it: nineth rib, right side); sternal nodule (probably a metaststic lymph node).
The good things are the lesions previously locally treated with surgery or sbrt (abdominal lymph node, diaphragm and of course liver) which even without a systemic therapy are all gone.
Now I guess a turn of the screw is needed... maybe folfoxiri and radio on the rib... then maintainance? Metronomic chemo? Immuno trial? Sbrt o rfa here and there where suitable...?
Pleura... gosh...

Since you are based in Europe I would suggest that you may also seek a second opinion from some university hospitals in Germany - maybe heidelberg.
Now u need to list possible therapies and treatment that can be repeated and those that can be repeated but only once more -
Keep the option that can be repeated only once and may not lead to cure - for later and other regional specific treatments which can be repeated several times can be tried for each region !
U need to sell this to the Dr's and the tumour board would be a good place to start !
I am sure that if we hang in here for few more years we may have a shot at longer OS with the immunitherapies showing promising results
