Ostomy Comprehensive Health and Life Assessment

If you are an adult (over the age of 18), and have had your ostomy surgery within the last 18 months, you are invited to participate in the Ostomy Comprehensive Health and Life Assessment survey. The rapidly changing fabric of health care in North American and world societies makes a comprehensive knowledge of the ostomate a necessity for health care providers and manufacturers of health care products. In order to maintain a dynamic understanding of the ostomy population, a statistically and scientifically rigorous quality of life, health, and health assisting needs survey is underway. This investigation recognizes that the ostomy population is a highly variable population with special needs and requirements. The investigation is seeking ostomates willing to complete an Ostomy Comprehensive Health and Life Assessment Questionnaire. The survey investigates various aspects of the life and health status, as well as the needs of the ostomate. By taking part in this study you will insure that your status as an ostomate is represented. You will be helping the global ostomy community by providing insight into the quality of life, health, and special needs of the ostomate population. Your participation will assist the sponsor of this research (Hollister Incorporated) in developing a global awareness of the quality of life, health, and needs of the ostomy population. Your participation is entirely voluntary, and there is no compensation for participation.
