O/T....anyone had hip bursitis and had cortisone shot?

Postby weisssoccermom » Mon Jul 23, 2018 12:40 am

O/T but need anyone's experiences, PLEASE!
Quick recap.....back in 2012/2013, I was trying to stack some wood, put too much in the wheelbarrow, it tipped over and I fell....HARD on the concrete in our garage....on the right side of my tush/hip and then hit my head. I was in PAIN and went to the ER. X-rays were taken....nothing appeared to be broken, etc. Since then, I have been in pain on/off. My muscles in my butt spasm and my hip area hurts. I've tried PT, OTC meds and even some Rx meds. Nothing works so I finally said, 'ok, I've got to do SOMETHING'. Last Dec, I had an MRI....nothing out of the ordinary. I've been treated for a myriad of issues....some PTs say 'its piriformis'..another says 'hip flexor muscle problem', etc. I saw a doctor who recommended I see this ortho rehab doc for ultrasound guided injections. My consult with her seemed to point to hip bursitis and NOT the other things I have been treated for all these years. Ok, scheduled my cortisone shot into my bursa and the tendon sheath for last Tuesday (July 17th). It went well...or so I thought. So here's where I need any advice or to hear your stories. While yes, I was always in 'pain' it wasn't like this!! Immediately after the shot...I was in Heaven until....the lidocaine wore off. Then this AWFUL burning/pulling feeling around the injection site started. Never had anything like that....ever! It was as though I had been burned and my clothes were scraping against me. Sometimes the burning felt more on the surface....others it was a deep down burning. While that pain has diminished somewhat, it is still there if I move the right way or honestly, I don't know what the heck I do/don't do to make it happen. It's been 6 days and it is still there.
Has anyone experienced anything like this? I NEVER had this type of pain prior to the shot. I've read a person can get something called 'cortisone flare' and I wonder if that might be what I am dealing with????

I'll call the office tomorrow but wanted to hear from anyone whose had a cortisone shot and had this type of a reaction. It's awful!

Yeah you know the drill, call your doc !!!

Wifey has had direct hip injection ( not into the external bursa ) with similar experience :
burning / painful for 2 days after lidocaine is gone BUTT...
6 days seems a little long for this ????
She was also on Celebrex starting a couple of days after, so that may have helped things.

Hope you're feeling better soon

Thanks for the info. I never had this burning pain before.
Called the doc this morning....haven't heard back from them yet.
I'm scheduled for another cortisone shot (after the wedding) in the ischial burse BUTT.....(literally) I'm not sure.
IF this is what they call the 'cortisone flare'....I don't know whether or not I want to go through it all over again :(

On a side note....I felt much better this morning BUTT....I took a 750mg methocarbamol last night. That helped stop the PAINFUL
glute spasms. I can't take that though during the day or I'll be out all day long!
Minor minor burning pain so far today...then again, I quit the Meloxicam and went back to 2 Ibuprofen X 3-4/times a day....at least for the next few days.

Hey CR...did your wife do a lot of icing afterwards. There was no way I could even put an ice pack next to that burn on day 1 and for 1/2 of day 2....did it for day 3 and it 'seemed' better but then I backed off the ice and burn came back. It IS less intense than on day 1 but I just wanted everything to be 'normal'!!

Not a lot of icing 2-3 x per day 2 days

I understand "flare" is more about the cortisone crystallizing in the tissues and your body is reacting,
whereas Intra-articular injections don't appear to have the same response ???

she also had others into her knee and did very well with those.
All given by sports medicine specialists and physiatrist, so not sure about the differing "techniques" used for injections
other than what I did in practice = sterile prep and SMALL needles with SLOOOOOOWW infusion rates

So this doc is a sports rehab/pain specialist and this is one of her specialties. My previous knee injections were not cortisone..they were either Hyalgen or Euflexxa (sp), so not the same. When this doctor did the bursa injection, she also put a tiny amount (at least that's what she said) into the sheath around the tendon and also told me that she saw some of the cortisone going backwards towards the glute muscles. Hoped that small amount would perhaps make a difference with the muscle spasms??? I wasn't sure if the 'flare' (I had read the same thing about the crystals) would affect the bursa or maybe the tendon or maybe the pain I'm feeling has absolutely nothing to do with the 'flare'.
It's better today but dang....this is brutal! I've had a burning feeling with neuropathy but man this burn is BAD!!! Waiting to hopefully hear back from them today.

Yeah she had both the cortisone and the hyaluronic acid injections to the knee,
different Docs at different times of course
the hyaluronic acid injections had temporary sensitivity but no after effects
like you are describing with the cortisone

Get your Doc on it and give us some feedback !!!!

Talked with doc's office late yesterday....around 4:30ish I think.
She said this is 'normal'....and yes, it might be a 'cortisone flare'.
Apparently the 'burning' is common ...wish I had known first...still would have done it.
Ice, rest, stay off stairs or other activities which would aggravate it.
Muscle relaxants are fine....actually they help when the surrounding muscles are cramping up in response to the pain. Took a 500mg methocarbamol and broke it in half..still got groggy....but at least I was staying at home...so no big deal.
Reality..it is getting better....some days the burning is a 2 compared with a 9 or 10 the day of/after the shot...so I guess that's a good thing???

Last thing...IF this lasts to 2 weeks....then I need to come in and see her. Today is 1 week and honestly, it is better. There was always the plan to get another shot in the ischial bursa....now....well. Asked if it was likely this would happen again and told ....YEP...my response.....YIKES!!

Thanks for asking. Hoping CR that it is not as hot up there for you guys. It hit 93 here yesterday....bright hot sun! NOT a fan BUTT....listening to what my kiddos are facing in Colorado and Philly (they have it really badly right now).....I'll take the hot dry heat ANYDAY over that! Hope that this weather pattern on the East Coast stops in time for the wedding!

This post has me concerned. My husband was scheduled for a cortisone shot in his hip with them using an X-ray to inject. We had to delay that when he ended up in the ER with all this colon stuff. Now he’s working on his 3rd chemo treatment. The pump comes off tomorrow. He’s been given the go ahead to get the shot. He was worried before. After reading this he is more worried. But his hip is bothering him so bad. On his CT report from the hospital they have his arthritis listed as severe or something like that for his age.

My shot was given into the bursa...not the hip joint. I can't tell you if there is anything different. Today, I am feeling much better. IMO, if a few days of intense pain garners me a better result in the long run, I'm willing to take it. Each person, however, has to decide for themselves. If his arthritis is that severe, why not give it a try???
I'm seeing the doc in two weeks for my follow up and am planning on receiving the shot into my ischial bursa next. I'm preparing for the same reaction but again, in the long run, if it even HELPS me (doesn't have to actually 'fix' the problem)...I'll be SO happy!

I want him to. He just likes to drag his feet. I finally got him to see an ortho last October. They said he should try a cortisone shot. It took him 6 months to decide to make the appt to have it done. It was scheduled for the Tuesday after he was released from the hospital. (Just by accident). We of course had to cancel because they wanted him to heal a bit. He got the go ahead a couple weeks ago and he keeps putting of getting his shot done.

If this is any help....I just bit the bullet and walked UP and DOWN the stairs to take out the dog. Now, that may not sound like a big deal but....for the past year, I have avoided the steps almost exclusively. Instead, I take the dog out through the garage where we have a ramp. Now, the other area in back of my hip still hurts a lot BUT....I've noticed today that the area directly into my hip that I had my cortisone shot is SO much better. Burning is pretty much gone....pain as well. Walking up and down the steps is a BIG deal to me.

BTW - I can't speak to the actual shot IN the hip but into the bursa was SO less concerning than I thought. Yeah, the lidocaine burns for a second but that's it. I've had years of hylagen shots into my knee joint and while everyone told me that would be way worse than the cortisone, I didn't believe them. Man, I wish I had. Like your hubs, I kept putting mine off...instead opting for PT, OTC meds, etc. Here I am, supposed to dance with my son at his wedding in less than 2 weeks and had I not been a jerk and kept putting it off, I could have had BOTH injections and been feeling way better. Now, I've got to go back to work right after Labor Day (teach at elementary school) so I am now pressed to get this next shot done, be prepared for the week of discomfort (may not happen) in order to feel better once school starts.

Moral of the story......just have hubs DO IT! Get it over with. He'll be glad he did.

Cortison/corticosteroid injections have mixed results. They are not a panacea for everyone.

Corticosteroids given as epidural injections for buldging discs have an uneven success rate. The clinical literature suggests that epidural corticosteroid injections have limited effect but more physicians are offering them due to the crack down on opiate prescriptions.

Corticosteroids can be overly activating for some people. Agitation, irritability, high burst of energy (like a “caffeine high”), mood swings, nerve sensitivity, increased pain sensitivity.

I asked him if he wanted his shot. He said yes he hasn’t had time to call. So I called and he has his appt this Friday at 8am. I’m glad they could make his appt so fast.

My suggestion...have him take some Tylenol or Ibufrofen before he goes.
Tell us how it goes.

ummmmmmm ...... maybe check with the Doc first
while acetaminophen is "usually" ok at normal doses, any liver issues and it would be contraindicated

NSAIDS and cortisone do not always play nicely together = if getting cortisone =>>> get SPECIFIC clearance before using an NSAID
( aspirin/ASA, ibuprofen, ketoprofen, naproxen and any of the COX inhibitors like Celebrex etc.)

CHECK with the docs before mixing meds

Cheers and best wishes
