May I recommend getting a PET scan as golden standard of imagery for any potential micromets getting into pertineum etc.

Order foundation 1 testing as soon as possible to confirm he is MSS or lucky enough to be MSI-H Chemo at moment looks best plan to stabilise situation, if he is MSI H that potentially a game changer so important to know. Would recommend repeat PET scan every 6 months reviewed by 2 different radiographers, if money was no issue would do this for 3years and revert to yearly PETs, 30% of CRCs spread to peri which cant be picked up on normal scans.

Be aggressive. Push very hard for HIPEC , try every HIPEC specialist centre and push push push.

Dont hang around and dont count on chemo for Peri stuff

It is much more likely you have colitis or crohns than cancer so stay positive and healthy

How accurate is foundation 1 / MSI-H testing?

Are there stats on the accuracy of these tests?

How important is the quality if tissue sample?

Could incorrect sampling occur ?

It got me thinking there must be some error and therefore some value in late stagd immunotherapy in MSS patients?

From a first hand perspective the symptoms are similar to a sbo.

Sbos can be managed.

Does he have extensive peri mets?

Venting g-tube and Tpn could provide symptom resolution and perhaps offer some time for chemo if desired.

If there is 25% success rate from this combo that is huge. It might not sound great on the face of it by Opdivo mono-therapy had less than 5% efficacy in MSS patients. This gives patients a big boost

Looks likely to be diffuse cancer causing blockage but no definitive answer yet. Still mostly blocked with NG tube in now for 5 days, complete bowel rest, TPN and just about managed pain.
